Course Description:
Nursing care focusing on chronic conditions of older persons including rehabilitative, palliative, hospice, and end-of-life care.
Course Objectives:
At the end of the course, students should be able to:
- Discuss the concepts and models on chronic care, ethical concerns, and practice settings.
- Analyze supportive care in terms of rehabilitative, palliative, hospice and end-of-life care.
- Demonstrate skill in performing comprehensive geriatric assessment
- Apply nursing process in the care of older persons with geriatric syndromes and chronic conditions.
- Demonstrate clinical decision-making skills using evidence-based information
- Plan with the patient and the family the support system needed to enhance care.
- Demonstrate dedication and compassion in providing nursing care.
