Course Description:

Nursing care focusing on chronic conditions of older persons including rehabilitative, palliative, hospice, and end-of-life care. 

Course Objectives:

At the end of the course, students should be able to:

1. Discuss the concepts and models on chronic care, ethical concerns, and practice settings.
2. Analyze supportive care in terms of rehabilitative, palliative, hospice and end-of-life care.
3. Demonstrate skill in performing comprehensive geriatric assessment
4. Apply nursing process in the care of older persons with geriatric syndromes and chronic conditions.
5. Demonstrate clinical decision-making skills using evidence-based information
6. Plan with the patient and the family the support system needed to enhance care.
7. Demonstrate dedication and compassion in providing nursing care.
